Literacy Reporting Form

Use this form to report statistics for your basic literacy classes using LEI primers and curricula per the LEI user agreement. Please submit this survey annually (once a year) for each primer language used. For example, if you have 3 literacy classes using the Spanish primer and 2 literacy classes using Firm Foundations, please fill out the survey once for the Spanish primer, submit and then fill out the survey again for the Firm Foundations classes and submit.

City in which the class meets.
State or province in which the class meets.
For basic literacy enter the language of the primer here. For UEE, PWE, Light of the World or Firm Foundations please enter the name of the curriculum. Please only submit one form for each language or curriculum.
How many classes (total groups of students, not individual teaching sessions) taught using this primer or curriculum?
Enter the total number of teachers for all classes for this primer or curriculum.
Enter the total number of students attending all classes for this primer or curriculum.
Enter the total number of new readers for this primer or Firm Foundations. ELM curriculum users enter "0".
